In response to the COVID-19 pandemic, and in the interest of keeping our patients, staff and volunteers safe, we have made the following changes to our operations and service provision until further notice:
The primary health care cliniccontinues to support Casa’s patients to ensure continuity of care. This includes addressing patient questions and concerns, education, and refill authorizations. Providers are reviewing charts and scheduled patients are offered telehealth services with a medical provider, an interpreter, and a staff medical assistant The primary health care clinic will offer telehealth services with a medical provider, an interpreter, and a staff medical assistant available to each patient.
At the Mental Health Collaborative (MHC), Casa and its MHC partners continue to offer mental health services via telehealth.
The GUIA Program—which specializes in advocating for patients and facilitating access to the regional healthcare system—is providing remote support to patients as they continue to navigate their health care needs. This includes ongoing coordination of urgent and non-urgent referrals, over-the-phone navigation services, increased patient education efforts, and continued assistance to patients with medical bills. The GUIA Program is also connecting patients affected by the pandemic to a variety of resources for food assistance, rent/utility assistance, and more.
